*Heart attack does not include established (old) myocardial infarction occurring prior to the issue date, sudden cardiac arrest, cardiac arrest or cardiopulmonary arrest.

Benefits will not be paid for a critical illness caused by a pre-existing condition unless the date of diagnosis is after the policy has been in force for 12 months from the issue date or last reinstatement date.
This product offers benefit amounts from $5,000 to $50,000.
The process is easy and straightforward. During the application process, you’ll need to provide basic personal information, including your social security number and answer some questions about your health history. You’ll also need to provide authorization for us to check third-party data sources to verify your identity and determine your eligibility for coverage. It’s important to answer all questions with complete honesty and transparency so your coverage will be there when you need it.
Each policy only insures one individual. If you and your spouse or partner wants coverage, you would each complete your own application.
Many applications are approved immediately at the time of application. Some decisions might take up to a few days while our experienced underwriters review the application.
Approvals for coverage are based on the applicant’s health history collected on the application and third-party data sources. Permanent U.S. residents aged 18-64 are eligible to apply for coverage. Full eligibility requirements include:
- Have comprehensive medical insurance
- Are a U.S. citizen or green card holder
- Live in one of these states: AL, AK, AZ, AR, AZ, CT, DE, GA, HI, IA, IL, IN, KY, LA, MD, MI, MN, MS, MT, NC, ND, NE, NJ, NV, NY, OH, OK, OR, PA, SC, SD, TN, TX, WA, WV and WY
- Don't have a disqualifying pre-existing condition listed during the application process
- Aren't replacing an existing critical illness insurance policy
Critical illness insurance claims submitted with the required documentation of diagnosis are typically processed and paid within 7-10 days.
Benefits are paid directly to you in the form of a lump-sum check. You can use the money however you wish.
